What Is ePHI (electronic Protected Health Information)?

DEFINITION

ePHI (electronic protected health information) is any individually identifiable health information that a HIPAA covered entity or business associate creates, receives, maintains, or transmits in electronic form. It spans data in EHR systems, databases, email, cloud storage, backups, and medical devices, and it is the specific class of information the HIPAA Security Rule's safeguards exist to protect.

ePHI is the electronic subset of protected health information under HIPAA. Information qualifies when it relates to a person's health condition, care, or payment for care and can identify the individual, whether through direct identifiers like names and medical record numbers or through combinations of data points. The format does not matter. ePHI lives in electronic health records, practice management systems, imaging archives, email and messaging, spreadsheets, cloud applications, backups, and connected medical devices.

The concept matters to covered entities and business associates because the HIPAA Security Rule's administrative, physical, and technical safeguards apply specifically to ePHI. An organization cannot scope those safeguards correctly without first knowing every system, vendor, and workflow where ePHI resides, which is why data inventory and system mapping sit at the front of any credible security risk assessment.

The practical implication is that ePHI usually spreads further than leadership assumes.
